These terms apply to SMS messages sent by Lim Family Medical Corporation, doing business as Bridgeview Primary Care.

By agreeing to receive SMS messages from Bridgeview Primary Care, you may receive informational and customer-care messages, including appointment reminders, appointment scheduling or changes, office notifications, requests to contact the office, and responses to questions or ongoing conversations.

Consent

Consent to receive SMS messages is not a condition of receiving medical care or purchasing goods or services. Where verbal consent is used, the practice may document the date, time, telephone number, purpose of consent, and staff member who obtained it.
